To set up your virtual weight loss consultation with our doctor, please complete the follow steps
Step 1 - Complete Registration
To register, click on the “To Registration” button below. We offer various VIP Club plans for our clients. Fill out the form and you will be registered to our FREE VIP Club Bronze level. Registration allows you securely complete forms and consents. Remember your password. You will need it.
By way of the Health Insurance Portability and Accountability Act of 1996 (HIPAA) the US government requires that we provide you notice of how we use your information. It describes your rights and your choices. It also describes our responsibilities.
We are a medical practice required by Boards of Medicine and by law to treat patients with medical best practices. One component is a medical history. Having complete and correct information helps providers to properly diagnose and treat medical conditions.
Step 4 - Receive Weight Loss Disclosure
The disclosure gives you information about weight loss and the use of GLP-1 medication. Clicking the button will send you the disclosure as a pdf file.
Step 5 - Make Appointment
On the appointment booking page: 1. select “Weight Control” 2. Select “Consult Online – Semaglutide” or “Consult Online – Tirzepatide” or if you are unsure which to choose select “Consult Online – Weight Loss”. 3. Select Provider “Logan” 4. Select your appointment date and time. 5. Enter your email (if you are registered your name and phone number will automatically appear). 6. Check the consent box. 7. Click on “Book”.
Step 6 - Payment
You may use this payment portal AFTER your consultation. Got to Step 7 now.
You can complete payment using the “To Payment” button below or call us at 813-343-8899. We accept almost all forms of payment but we do not accept insurance.
Semaglutide is $250 per four weeks. Tirzepatide is $450 per four weeks. Both are plus shipping by UPS and handling for $15. No other hidden costs. These costs are paid after the consultation if you wish to proceed.
On the day of your appointment you will receive a Zoom meeting invitation by text and/or email. Click on the link to attend the meeting or enter the zoom meeting credentials by clicking on the “To Zoom” button. If you have any problems connecting to the meeting call us at 813-343-8899.
If you have an iPhone or an Android phone and cannot connect using Zoom you can do the consultation with FaceTime. We will send you a text with a number to call for a FaceTime meeting.
